New Patient Information
Forms
If you are a new patient to our office, our forms are now digital. Our new patient coordinators
will email or text a unique link for your new patient forms approximately one week prior to your
scheduled appointment. We ask that you click the link and complete them upon receipt so we
can start your appointment promptly upon your arrival which will allow us to attend to your
medical needs more quickly than completing them on your arrival. If you have difficulties with
the link we send, a copy of each form is available linked below for you to print and complete
prior to arriving for your scheduled appointment. Thank you and please call our office if you

Payment Options
For your convenience, we accept cash, personal checks, money orders and major credit cards. Payment is expected at the time services are performed. When more extensive dental care is necessary, financial arrangements are available through CareCredit.
Insurance
We are in-network with Delta Dental Premier & HRI Dental (also known as Paramount Dental). We will file most insurance that allows out-of-network benefits. We will assist patients with insurance research and any necessary pre-determination of benefits. We will file pre-determination forms whenever possible.
As a courtesy, we will file your insurance claims. However, any estimated patient balance due is expected at the time of service.
We do not accept Medicaid or Medicare.
